**Form #1 must be filled out in full before each season a student is playing a sport. It must be turned in to the school nurse.

**Form #2 is used for a physical being done at the school. It must be filled out and signed by a parent.

SPORTS PHYSICALS FOR 9th - 12th GRADE STUDENTS PLANNING ON PLAYING 2013-14 FALL SPORTS 

 
 
WHO:         9th - 12th grader planning on playing Fall Sports
DATE:        Monday, August 12th
TIME:         9:00 a.m. (First Come First Serve) 
WHERE:     Niagara Wheatfield High School Large Gym  
 
  
 

      All District sports physicals will be performed by the staff of physicians and nurse practitioners from University Sports Medicine of Niagara.

  
      If your child plans on participating on a JV or Varsity athletic team, in the upcoming 2013-14 school year, they must obtain a physical during the above time.
 
      If your child has a history of asthma, severe allergy/epi-pen, or uses an inhaler, a Health Care Plan and Medication Form must be completed by your family physician and returned to the Health Office by August 1st,
 or prior to the beginning of fall sports. These forms can be picked up in the Health Office, or downloaded from
the school website. Should the condition no longer be a problem, please send in medical documentation so that your child’s health file can be updated.
 
      Every student must have a completed and parent signed copy of the attached “Pre-participation Physical Evaluation” form. The student should return this form to the Health Office prior to the above dates.
 

      Students who wear glasses or contacts must have one of them with them at the time of the physical.

 
 
SPORTS PHYSICALS: 2013-2014 SCHOOL YEAR  REQUIREMENTS FOR SPORTS PHYSICALS
  • "Pre-Participation Physical Evaluation" form must be completed on both sides and signed by parent(s)/guardian(s).
  • "Health Care Plan" completed and signed by parent(s)/guardian(s) and/or physician
     (if applicable).
  • Glasses or contacts for vision test, if required, for distance vision.
  • Medical Clearance - If your child was not in Physical Education classes or has been on limitations, a note from the treating physician must be submitted stating clearance to return to gym/sports.

FAILURE TO SUBMIT ALL REQUIRED PAPERWORK WILL PREVENT ATHLETES FROM STARTING PRACTICE IN A TIMELY MANNER.
 
*****These Sports Physicals meet the requirements for the mandated physicals that students need for entering 7th and 10th grades and do not have to be repeated.*****
